Cluster: Live Together, Mourn Together
This project is named PRALEiN, which derives from “talking about the end of life” (in Dutch) with elderly care recipients with a severe multiple (intellectual) disability. Healthcare professionals experience barriers, mainly due communication problems, in identifying the need for Care Planning for severe multiple-disabled recipients in the palliative phase. Recently, the Proactive Care Planning Discussion Guide has been developed, which is applied to care recipients with dementia and forgetfulness. Proactive Care Planning focuses on early mapping of life goals and choices for now and in the future. It promotes the person’s own life choices and the quality of life. This project focusses on testing and finishing of a concept Discussion Guide tailor made to elderly care recipients with a severe multiple (intellectual) disability.
